Formal Observations
This is a very open-ended form within the evaluation system meant for scripting a formal observation. It begins with a box for notes from the pre-conference and post-conference which the administrator will fill in. At the end of the formal observation the administrator will have a series of many notes, each time-stamped so they and the teacher can see at what point in time in the lesson that event occurred. This allows the administrator to be very specific about what practices they were seeing over the course of a long observation.
Informal Observations
Informal observations can take place at any time. They usually last 5 to 10 minutes. Currently the standard forms used for these types of observations are the Classroom Snapshot and the General Walkthrough - both are downloadable here

Teachers can also upload their own Snippets into the evaluation system that they believe enhance their portfolio of good practice. Teachers are encouraged to do this in a thoughtful manner that not only helps them to focus on growing good practice, but also helps to better inform the administrator when filling out their evaluation.
- A picture of a bulletin board or project from a classroom
- A picture of student work
- A news article
- A parent letter or email
- Student accomplishments attributable to a teacher’s classroom activities
- Awards or recognitions
- Links to work a teacher does online such as a classroom blog or website
- Descriptions of professional development a teacher has delivered
- Copies or links to professional presentations the teacher has done

Feedback Forms
Students and parents are a part of the evaluation process. They contribute feedback to a teacher's evaluation.
